Importance of Primary Care/Pharmacist Partnerships

SESSION DESCRIPTION:

Studies have demonstrated the benefit of a primary care/pharmacist partnership in the management of chronic disease, such as heart failure and arrhythmias. Among the benefits of this partnership are decreased occurrence of medication-related problems, adverse drug reactions, and medication errors. This session will explore the benefits of partnering with pharmacy in patient-centered care.

LEARNING OBJECTIVES:

  • Describe the key elements of Medication Therapy Management (MTM)
  • Identify areas of patient care in your practice that can benefit from pharmacist provided medication therapy management
  • Explain the various medication and disease related patient care services pharmacists provide
